Abstract


Patients on dialysis are in a state of immunodeficiency, resulting in a high incidence of infectious complications. These patients are a population at risk for developing infection or reactivation with cytomegalovirus (CMV). The objective of this study is to determine the prevalence of CMV antibodies (IgM and IgG) among hemodialysis patients treated in CHU Ibn Rochd in Casablanca. Patients and Methods: This transversal study of 87 hemodialysis patients and 30 controls was to search CMV antibodies (IgM and IgG) and assess risk factors of infection. The control population was consisting of blood donors in blood transfusion center in Casablanca. Results: The mean age of patients was 44.1

Les patients sous dialyse sont en général dans un état d'immunodéficience, aboutissant à une incidence élevée de complications infectieuses. Ces patients constituent une population à risque de développement d’infections graves, dont la réactivation, à Cytomégalovirus (CMV). L’objectif de cette étude est la détermination de la prévalence des anticorps (IgM et IgG) anti-CMV chez les patients hémodialysés du service de Néphrologie, Dialyse et Transplantation Rénale, du CHU Ibn Rochd de Casablanca. Patients et méthodes: Cette étude transversale est réalisée sur 87 patients hémodialysés et 30 témoins. Elle a consisté à rechercher les anticorps, IgM/IgG anti-CMV, et à évaluer certains facteurs de risque de l’infection par le CMV. La population témoin est constituée de donneurs de sang marocains du centre régional de transfusion sanguine de Casablanca. Résultats: La moyenne d’âge des patients était de 44,1 ans ± 12, avec une durée moyenne de dialyse dépassant cinq années chez 80,5 %. La moyenne d’âge chez les témoins était de 35 ans ± 9. La prévalence des IgM anti CMV était de 6,9% chez les patients et de 6,7% chez les témoins (p=0,7027). Celle des IgG anti-CMV était de 100% chez les patients et de 96,7% chez les témoins (p=0,5852). Conclusion: Vu les risques de complications sévères qu’encourent les patients hémodialysés lors d’une primo-infection ou une réactivation endogène du Cytomégalovirus, la recherche des marqueurs de CMV (IgM et IgG) chez les hémodialysés doit être systématique. Par ailleurs, la transfusion sanguine est encore considérée comme un facteur de risque majeur d’infection à CMV. La déleucocytation reste la seule alternative pour la prévention de l'infection post-transfusionnelle.
