Abstract


Helicobacter pylori and the others since the discovery of H. pylori, several new Helicobacter species have been isolated from man and mainly from animals. Helicobacter species can be broadly grouped according to whether they colonize the gastric or enterohepatic niche. H. pylori is a bacterium of great clinical importance, essentially in the domain of gastroenterology. H. pylori infection is the first chronic infection known to give rise to cancer in man (gastric carcinoma and MALT lymphoma). The pathogenesis of H. pylori is now well studied. Two H. pylori strains have been sequenced: the first one isolated from a patient with gastric ulcer, the second one associated with gastritis. Global analysis of the gene content of H. pylori strains gives insight into the extent of its genetic diversity. Substantial evidence attests to certain extragastric Helicobacter species playing a role in the pathogenesis of enteric, hepatic and bilary disorders, especially for H. hepaticus which have just been sequenced. But isolation of non-pylori Helicobacter species continues to be a major problem, substantially limiting a better understanding of their prevalence and role. Therefore, animal modsels are of interest because of their value for modeling human disease and testing therapeutic strategies such as vaccines.

Helicobacter pylori est sans conteste le chef de file des bactéries du genre Helicobacter. De nombreuses espèces sont maintenant caractérisées et classiquement divisées en deux grands groupes: les Helicobaters gastriques et les Helicobaters entérohépatiques. Dans ces deux groupes, seules certaines espèces ont pu être isolées chez l'homme, les autres étant uniquement rencontrées chez les animaux. La pathogénicité de H. pylori est maintenant bien étudiée. H. pylori est responsable chez l'homme de gastrite, d'ulcère et de deux types de cancer: le carcinome gastrique et le lymphome du MALT. Deux souches ont été séquencées, l'une isolée d'un patient atteint d'ulcère, l'autre associée à une gastrite. Une des caractéristiques la plus frappante de H. pylori réside dans sa diversité génétique. Le rôle des Helicobaters entérohépatiques dans les maladies hépatobiliaires commence à être bien documenté, en particulier pour H. hepaticus dont le génome vient d'être séquencé. À l'inverse, nos connaissances sur la pathogénicité d'autres espèces se heurtent à la difficulté de les isoler et donc d'accéder à leur information génétique. Cependant, des modèles animaux d'infection nous permettent de disposer d'outils pour évaluer une infection naturelle par des bactéries du genre Helicobacter, voire de tester des vaccins anti- Helicobacter.
