Abstract


The parietal endometriosis is an uncommon clinicopathological variety, its pathophysiology remains unknown. It usually occurs after pelvic surgery. We report the case of a patient who presents a cyclical pelvic pain, sitting at the caesarean scar. With the examination of a 4cm mass in the right iliac fossa. The Doppler ultrasound showed a polylobed image, tissue, center vascularization, measuring 40/24 mm, suggestive of parietal endometriosis or a tumor of soft parts. So we said a wide resection of the lesion, in which were excised a solid mass of 3.3 / 3 cm under the fascia adherent to the rectus muscle of the abdomen. Histological study objectified diagnosis of endometriosis of the wall. The postoperative course was simple with a decline of 16 months without recurrence. This is a condition that remains mysterious to the interest or understanding of its characteristics to improve prognosis by diagnosis, early treatment and prevention during each surgery.

L'endométriose pariétal est une variété anatomo-clinique rare, sa physiopathologie reste inconnue. Elle survient généralement suite à une intervention chirurgicale pelvienne. Nous rapportons l'observation d'une patiente qui présente une douleur pelvienne cyclique, siégeant au niveau de la cicatrice de césarienne. Avec à l'examen une masse de 4cm au niveau de la fosse iliaque droite. L'échographie-doppler a montré une image polylobée, tissulaire, à vascularisation centrale, mesurant 40/24 mm, pouvant évoquer une endométriose pariétale ou une tumeur des parties molles. Donc on a indiqué une résection large de la lésion, au cours de laquelle on a excisé une masse solide de 3,3/3 cm, sous l'aponévrose adhérente au muscle grand droit de l'abdomen. L'étude anatomopathologique a objectivé le diagnostic d'endométriose de la paroi. Les suites postopératoires étaient simples avec un recul de 16 mois sans récidive. C'est une pathologie qui reste mystérieuse d'où l'intérêt de mieux comprendre ses caractéristiques pour améliorer le pronostic par un diagnostic, une prise en charge précoce et de sa prévention au cours de chaque chirurgie.
