Abstract


Objective: To evaluate the correlation between total anti-Hbc antibody seropositivity and anti-nuclear antibody seropositivity, in order to prove an association between hepatitis B virus infection and the pathogenesis of autoimmune diseases. Material and methods: Retrospective case-control study, conducted in the immunology department of CHU Ibn Rochd of Casablanca from January 2017 to January 2022, evaluating the results of the analyses of 1099 patients, in whom a search for antinuclear antibodies was simultaneously carried out with the search for Hbs antigen and total anti-Hbc antibodies. The patients were divided into two groups. A control group with 937 Hbs antigen and Hbct antibody negative patients and a case group with 162 patients positive for total anti-Hbc antibody. Testing for antinuclear antibodies was performed by indirect immunofluorescence on slides sensitised with Hep-2 cells. Hbs antigen and total anti-Hbc antibodies were tested by automated immunochemiluminescence. Results: We obtained a seroprevalence of anti-nuclear antibodies of 40.75% in the case group and 22% in the control group (P <0.0001). No statistically significant difference between the two groups in the frequency distribution of fluorescence patterns in antinuclear antibodies positive patients was observed (P = 0.617). Conclusion: Our study, in correlation with various literature data, affirms an established association between hepatitis B virus and various autoimmune diseases.

Objectif: Evaluer la corrélation entre la séropositivité des Anticorps anti-Hbc totaux et des anticorps anti-nucléaire, afin de prouver une association entre l’infection par le virus de l’hépatite B et la pathogénèse des maladies auto-immunes. Matériel et méthodes: Etude rétrospective cas-témoins, menée dans le service d’immunologie CHU Ibn Rochd de Casablanca du janvier 2017 au janvier 2022, évaluant les résultats des analyses de 1099 patients, chez lesquels, une recherche d’anticorps antinucléaires a été simultanément réalisée avec la recherche de l’antigène Hbs et des anticorps anti Hbc totaux. Les patients ont été divisés en deux groupes. Un groupe témoin comportant 937 patients négatifs pour l’antigène Hbs et les anticorps Hbc totaux et un groupe de cas rassemblant 162 patients possédant des Anticorps anti-Hbc totaux positifs. La recherche des anticorps antinucléaires a été effectuée par immunofluorescence indirecte sur des lames sensibilisées par des cellules Hep-2. La recherche de l’Antigène Hbs et des Anticorps Hbc totaux a été effectuée par immuno-chimiluminescence automatisée. Résultats: Nous avons obtenu une séroprévalence des anticorps antinucléaires de l’ordre de 40,75% chez le groupe de cas et de 22% chez le groupe des témoins (P <0.0001). Aucune différence statistiquement significative entre les deux groupes, en ce qui concerne la répartition des fréquences des aspects de fluorescence chez les anticorps antinucléaires positifs, n’a été observée (P = 0.617). Conclusion: Notre étude, en corrélation avec diverses données bibliographiques, affirme une association établie entre le virus de l’hépatite B et diverses atteintes auto-immunes.
