Abstract


Endometriosis is classically defined by the ectopic presence of tissues possessing the morphological and functional characteristics of the endometrium. Ovarian endometriosis is one of the most common locations. Its pathogenesis remains unclear, but a multifactorial origin has been proposed. The symptomatology is dominated by pain and infertility. The clinical examination is often poor. The suprapubic pelvic ultrasound, and especially the endovaginal ultrasound, remains the examination of first intension. Laparoscopy is the only test to affirm with certainty; biopsies, classify it into stages and choose the most appropriate therapeutic strategy. The treatment of the first intension remains surgical, the medical treatments being in the majority of the cases only palliatives. The therapeutic success depends on the radicality of the surgical excision. We report the case of a postmenopausal patient with ovarian endometriosis simulating ovarian neoplastic pathology with review of the literature.

L’endométriose est classiquement définie par la présence ectopique de tissus possédant les caractères morphologiques et fonctionnels de l’endomètre. L’endométriose ovarienne est l’une des localisations les plus fréquentes. Sa pathogénie reste mal élucidée, mais une origine multifactorielle a été proposée. La symptomatologie est dominée par la douleur et l’infertilité. L’examen clinique est souvent pauvre. L’échographie pelvienne sus-pubienne et surtout endovaginale demeure l’examen de première intension. La coelioscopie est le seul examen permettant d’affirmer avec certitude ; grâce aux biopsies ; la présence de cette affection, de la classer en stades et de choisir la stratégie thérapeutique la mieux adaptée. Le traitement de première intension reste chirurgical, les traitements médicaux n’étant dans la majorité des cas que palliatifs. Le succès thérapeutique dépend de la radicalité de l’exérèse chirurgicale. Nous rapportons le cas d’une patiente ménopausée porteuse d’une endométriose ovarienne simulant une pathologie néoplasique ovarienne avec revue de la littérature.
