The systematization of ultrasound in the 1st trimester with a search for the nuchal translucency has made it possible to detect numerous pathologies, in particular cystic hygroma, the prevalence of which can reach 2%, and which is defined as having a retro and laterocervical poly cystic mass, These cystic formations are composed of multiple compartments of varying size and containing a liquid; and they are microscopically defined as new cavities formed in the connective tissue and lacking a classic cystic wall, that is to say, they are pseudo-cysts without an internal coating, hence the interest in differentiating them from cervical cystic lymphangioma which has a lymphatic endothelial type coating of variable volume, related to sequestration of lymphatic fluid in the nuchal connective tissue. And which is generally unilateral left anterolateral, and can also involve the shoulder.And also differentiating them from the occipital meningocele by the demonstration of a defect of the bone closure at the level of the cervical spine.Cystic hygroma is often associated with karyotype abnormalities of up to 80% made of monosomy X or trisomy 21 or others, as well as other morphological abnormalities; cardiac, diaphragmatic, skeletal or a set of syndrome. A good knowledge of the factors of poor prognosis makes it possible to inform prenatal counseling and set up correct management.

La systématisation de l’échographie au 1er trimestre avec recherche de la clarté nucale a permis de déceler de nombreuses pathologies notamment l’hygroma kystique, dont la prévalence peut atteindre 2%, et qui est définit comme étant une masse poly kystique rétro et latérocervicale, Ces formations kystiques sont composées de multiples logettes de taille variée et contenant un liquide; et il s'agit microscopiquement de cavités néoformées dans le tissu conjonctif et dépourvues de paroi kystique classique, c'est-à-dire, ce sont des pseudo-kystes sans revêtement interne, d'où l'intérêt de les différencier du lymphangiome kystique cervical qui possède un revêtement de type endothélial lymphatique de volume variable, en rapport avec une séquestration de liquide lymphatique dans le tissu conjonctif nucal, et qui est en général unilatéral antérolatéral gauche, intéressant aussi l'épaule.Mais aussi du méningocèle occipital par la mise en évidence d'un défaut de la fermeture osseuse au niveau du rachis cervical.L’hygroma kystique est souvent associé à des anomalies du caryotype allant jusqu’à 80% fait de monosomie X ou trisomie 21 ou autres, ainsi qu’à d’autres anomalies morphologique, cardiaque diaphragmatique, squelettique ou un ensemble de syndrome. La bonne connaissance des facteurs de mauvais pronostic permet d’éclairer le conseil prenatal et mettre en place une prise en charge correcte.
