[ Tumeur rétropéritonéale rare - Schwannome bénin associé à la maladie de VON RECKLINGHAUSEN : A propos d’un cas avec revue de la littérature ]

Schwannoma is a benign nervous tumor arising swhan cells with a difficult preoperative diagnosis. The retroperitoneal localization is exceptional and its occurrence during Von Recklinghausen’s disease is rare. Von Recklinghausen’s disease is an evolutionary pathology characterized by the involvement and impairment of several organs deriving from the neural crest. Some complications can be seen, but the most serious complication is the degeneration of neurofibromas. We report the case of a 28 years old patient, who consulted for left low back pain, CT scan has revealed a cystic mass behind the left hepatic lobe, and MRI was in favor of a remanied bilary cyst. a laparoscopy for diagnostic and therapeutic purposes has been proposed, and has objective a retroperitoneal cystic mass, that doeas not depend on the liver or the pancreas, pathologic examination after total excision of the tumor, conclude with a benign schwannoma, subsequently the diagnosis of the associated Von Recklinghausen disease was established before the combination of diagnostic criteria. Complete surgical excision of the tumor the reference treatment. Recurrence and transformation, altrough rare after surgery, requires post-operative monitoring by an annual CT scan.

Le schwannome est une tumeur nerveuse bénigne, de diagnostic pré-opératoire difficile, sa localisation rétropéritonéale est exceptionnelle et son apparition au cours de la maladie de Von Recklinghausen est rare. La maladie de Von Recklinghausen est une pathologie évolutive caractérisée par l’atteinte de plusieurs organes dérivant de la crête neurale. Des complications peuvent se voir, mais la complication la plus redoutable est la dégénérescence des neurofibromes. Nous rapportons le cas d’une patiente de 28ans ayant consulté pour des lombalgies gauches. La TDM avait mis en évidence une masse kystique en arrière du lobe gauche hépatique et l’IRM était en faveur d’un kyste biliaire remanié. Une cœlioscopie à visée diagnostique et thérapeutique a été réalisé et a objectivée une masse kystique rétropéritonéale qui ne dépend ni du foie ni du pancréas. L’examen anatomopathologique après exérèse totale de la tumeur a conclu à un shwannome bénin. Par la suite le diagnostic de la maladie de Von Recklinghausen associé a été établi devant l’association des critères de diagnostic. L'exérèse chirurgicale complète de la tumeur est le traitement de référence. La récidive et la transformation bien que rare après chirurgie impose une surveillance post opératoire par tomodensitométrie annuelle.
