Abstract


Introduction: Cystic lymphangioma of the ovary is a rare benign vascular tumor. Its histogenesis is still hypothetical and its preoperative diagnosis difficult to establish. We report a case of cystic lymphangioma of the ovary that was a problem with differential malignant ovarian tumor diagnosis, with a recent review of the literature.
Observation: Patient 35 years primigravida primipara without significant pelvic pain consults history dating back more than three months without increased abdominal volume or urinary or digestive signs associated. A pelvic ultrasound done complemented by magnetic resonance imaging have objectified aspect for a right ovarian cyst formation by 8 cm to confront the histological data. The patient underwent a right oophorectomy returned for mature cystic teratoma of a cystic hygroma associated. The postoperative course was uneventful and no recurrence was noted.
Results: Cystic lymphangioma of the ovary is rare benign tumor formation. There are no specific symptoms. Radiology despite progress remains inconclusive in the diagnosis of this tumor type. Histological study is the only way to confirm the diagnosis. The outcome was favorable, no cases of malignant transformation have been reported.
Conclusion: Cystic lymphangioma of the ovary is a rare benign tumor of the malformation. The diagnosis rarely made preoperatively should be considered in any cystic ovarian mass. Only complete surgical resection of the tumor is associated with prolonged survival and a favorable prognosis.

Introduction: Le lymphangiome kystique de l'ovaire est une tumeur vasculaire bénigne rare. Son histogenèse est encore hypothétique et son diagnostic préopératoire difficile à établir. Nous rapportons un cas de lymphangiome kystique de l'ovaire qui a posé un problème de diagnostic différentiel avec une tumeur maligne de l'ovaire, avec une revue récente de la littérature.
Observation: Patiente de 35 ans primigeste primipare sans antécédents notables consulte pour douleurs pelviennes remontant à plus de 3 mois sans augmentation du volume abdominal ni signes urinaires ou digestifs associés. Une échographie pelvienne faite complétée par une imagerie par résonance magnétique ont objectivées un aspect en faveur d'une formation kystique ovarienne droite faisant 8 cm à confronter aux données histologiques. La patiente a bénéficié d'une ovariectomie droite revenue en faveur d'un tératome kystique mature associé un lymphangiome kystique. Les suites opératoires étaient simples et aucune récidive n'était notée.
Resultats: Le lymphangiome kystique de l'ovaire est en formation tumorale bénigne rare. Il n'y a pas de symptomatologie spécifique. La radiologie malgré les progrès reste non concluante dans le diagnostique de ce type de tumeurs. L'étude anatomopathologique reste le seul moyen pour confirmer le diagnostique. L'évolution est favorable, aucun cas de transformation maligne n'a été rapporté.
Conclusion: Le lymphangiome kystique de l'ovaire est une tumeur malformative bénigne rare du. Le diagnostic rarement fait en préopératoire devrait être évoqué devant toute masse kystique de l'ovaire. Seule l'exérèse chirurgicale complète de la tumeur est associée à une survie prolongée et à un pronostic favorable.
