Abstract


Adenoid cystic carcinoma of Bartholin’s gland is a rare tumor. Its management is inspired by the treatment of vulvar cancers. We present here a clinical case of a 28-year-old female patient who presented with a painful swelling of the left labia majora, which was mistaken for simple bartholinitis and treated medically without improvement. A biopsy was then performed, and the histopathological study revealed a cystic adenoid carcinoma of the Bartholin gland. The radiological workup showed a locally advanced left vulvar process, without distant metastasis. Due to the impossibility of surgery, concomitant radiotherapy and chemotherapy were performed, with good improvement. The rarity of this type of vascular cancer makes its diagnosis late, because it is often confused with bartholin gland cyst and bartholinitis. Moreover, its management is still not standardized.

Le carcinome adénoïde kystique de la glande de Bartholin est une tumeur rare. Sa prise en charge est inspirée du traitement des cancers vulvaires. Nous présentons ici un cas clinique d’une patiente de 28 ans qui a présenté une tuméfaction douloureuse de la grande lèvre gauche, qui a été prise pour une simple bartholinite et traitée médicalement sans amélioration. Une biopsie a été alors réalisée, et l’étude histopathologique a révélé un carcinome adénoïde kystique de la glande de Bartholin. Le bilan radiologique a montré un processus vulvaire gauche localement avancé, sans métastase à distance. Vu l’impossibilité chirurgicale, une radiothérapie et chimiothérapie concomitantes ont été réalisées, avec une bonne amélioration. La rareté de ce type de cancer vulvaire rend son diagnostic tardif, car souvent confondu avec le kyste de la glande de bartholin et la bartholinite. De plus sa prise en charge n’est toujours pas standardisée.
